The Role:

Reporting to the Head of Care and working closely with the care team to implement and maintain a high-quality domiciliary care service throughout the retirement village.

The Care Coordinator will lead and manage their care team ensuring a high quality of care and service is provided to all service users at all times. As the care coordinator you will be responsible for rota planning and briefing the care team prior to their visits, conducting supervisions and managing customer visits when required.  

The Care Coordinator will also support the Head of Care with the recruitment of new carers, conducting interviews and managing clinical governance to ensure the delivery of safe, personalised services to each individual service user through assessment, person centred planning and regular outcome focussed reviews of services.

 

 

You will:

  • To promote the smooth day-to-day operations of the service and ensuring compliance with all requirements of the Care Quality Commission through rostering, recording and reporting in line with the company’s policies and procedures.
  • Preparing staffing rotas to ensure continuity in the delivery of care to service users. Ensuring these rotas, and any changes, are properly communicated to customers and care workers. Re-allocating shifts as necessary, to cover staff absences.
  • To support with regular review of the service users’ care needs and wishes to ensure on-going support is of the highest standards.
  • Investigating complaints, taking appropriate remedial action and if required reporting this to the Head of Care, Senior manager and/or the Care Quality Commission.
  • To ensure that health all requirements of Health and Safety are met and the Head of Care is aware of any incidents or short falls in safety.  

You will be:

  • Qualified in a NVQ/QCF Level 3 in Health & Social Care.
  • Experienced in a care position for a minimum of 2 years.
